  • Application for Passport by Mail Form DS-82 *
    (1 copy completed and signed)
  • Most recent passport
  • Two passport photos
  • Proof of name change
    (if applicable)
  • Letter of Authorization
  • Proof of departure
  • Check to “US Department of State” in the amount of $127

    • Print list of acceptable documents.

    * If your passport was issued more than 15 years ago or you were younger that 16 when it was issued, use the TravelFlex new passport service.

    2. Choose Emergency, Rush, or Budget service.
    3. Continue shopping or go through checkout

    Place your sealed envelope inside another envelope.

    Ship to TravelFlex by FedEx.
