How to Replace a Lost or Stolen Passport

You must report the loss of the passport to the US State Department immediately. To report a lost or stolen passport, follow the steps at the bottom of this page.

Check to “US Department of State” in the amount of $127 *

Print list of acceptable documents.

Bring documents to official acceptance agent near you. http://iafdb.travel.state.gov

The acceptance agent will seal an evelope containing your

check for $127 payable to US Department of State

Pay Acceptance Agent fee of $30.00.

Take the envelope with you.

• Fill in as much of the passport information as you can.
• Answer all the other questions in detail.

• Sign and submit one copy of Form DS-64, Statement Regarding a Lost or Stolen Passport, to:
